- How do I order a transcript?
The ordering process starts at ordertranscript.wisc.edu.
Currently enrolled students can access the System by logging in through My UW-Madison (MyUW) http://my.wisc.edu. If unable to log in or don’t remember your NetID and/or password, see the "Troubleshooting" section item number 5 for more information about assistance.
Former students can access the system by logging in through MyInfo http://myinfo.wisc.edu. If unable to log in or don’t remember your Campus ID and/or PIN, see the "Troubleshooting" section item number 5 for more information about assistance.
If you ordering transcripts, and unable to contact us during business hours to receive your log in information, you can enter the system without logging in. Simply choose ‘Start your transcript order now’. This option requires you to send us your signature. This will delay the processing of your order. If you are unable to order on-line, you may print out a hard copy Transcript Request form, fill it in completely, sign, And mail it to us with payment to the address on the form. This form is available at http://ordertranscript.wisc.edu.
- Can I order my transcripts over the phone, by email, or by fax?
Because of federal privacy regulations (FERPA), transcripts cannot be ordered over the phone, by email or by fax. (See the FERPA section for more information.)
- Can I make changes to my transcript order once it has been submitted?
No. However, we have provided ways to help you through the system to ensure you are successful in ordering what you want. We provide summary and confirmation screens throughout the process so you can verify the information is correct. You are also able to revisit previous screens in the ordering program to make adjustments. In sum, we have designed the system to allow you to adjust the order before committing to payment and processing.
- How many transcripts can I request?
You may order as many transcripts as you want. See cost chart for related charges for fees and quantity rates.
- Will you complete and include additional paperwork that needs to accompany my transcript order?
If you have any supplemental forms that must accompany your transcript (i.e., AMCAS or LSAC application forms, graduate school forms, etc.), you must select and complete the "Third Party Attachments" option when requesting your transcript. Please note that it may take longer to process your request when selecting this option, and that we will not begin processing your order until the attachment has been received in our office.
- Can I order my child’s transcript?
Access to a student’s academic record (including transcripts) is available only to the student. Students can designate any recipient to receive their transcript and/or forward their transcripts to whomever they choose.
- Can I request the transcript of a former student who is now deceased?
Yes you may. Please use the Transcript
Request Form and mail it in with an explanation
of your request, and include a death certificate or newspaper obituary as proof
of death. Please note: it might take longer to process these requests.
*Please provide this proof if the death was recent (within the past year) or
for a former student who was a non-citizen of the U.S.
- How does a third party order my transcript?
The simplest and quickest way would be for you to order the transcripts yourself and designate the 3rd party as the recipient.
